Student Evaluation

Please Note:

• The Student Questionnaire is to be completed by the student in his/her handwriting
• Two Student Evaluation Forms are to be completed by two different administrators, counsellors or teachers who have known the student for at least one year
• Send school records from the previous three years, including the most recent/current available records
• The results of any Standardised Testing
• For students who require Special Resources, please send any available information including Educational Evaluations and Individual Educational Plans.
